Incident & Near-Miss Reporting
Operators can quickly record accidents, near misses, chemical spills, and environmental incidents directly within OPS-APP. Using structured digital forms, the app captures all relevant details, including:
- Incident type and location
- Descriptions and contributing factors
- Photos and video evidence
- Immediate actions taken
Once completed, the form is digitally signed by the operator and submitted instantly.
From Reporting to Corrective Action
OPS-APP turns safety reporting into measurable action. Management can create workflows directly from submitted incident forms to request corrective or preventative actions.
All data is securely stored in your OPS-APP database for compliance, auditing, and continuous improvement.
Workflow Example
An operator records a near miss in a warehouse, where unstable stacked boxes narrowly avoided falling onto personnel working below. The near-miss report is submitted via OPS-APP with photos and a detailed description.
Upon receiving the notification, the safety team converts the near-miss report into a work order, requesting a full warehouse safety audit to assess racking, stacking methods, and storage procedures.
Once the audit is completed using OPS-APP, findings are recorded digitally and corrective actions are recommended, such as changes to storage layouts, improved stacking controls, or additional training. All actions, recommendations, and outcomes are captured within the platform, creating a clear, auditable trail from near-miss identification through to corrective action and continuous improvement.
Lockout / Tagout (LOTO) & Asset Status Visibility
OPS-APP also strengthens operational safety by providing clear visibility of equipment status and isolation events.
Digital LOTO Recording
When an operator carries out Lockout / Tagout in line with your SOPs, the entire process can be recorded within OPS-APP. Once LOTO is applied:
- Relevant maintenance personnel, SCADA operators, and managers are automatically notified
- The asset is marked as out of service
- Estimated downtime can be communicated instantly
Real-Time Asset Status Updates
When LOTO is removed, OPS-APP notifies all relevant personnel that the asset is back online, ensuring everyone is working from the same, up-to-date information and reducing the risk of unsafe re-energisation.
QR Code Access for Simplicity
LOTO equipment can be labelled with OPS-APP QR codes, allowing operators to instantly access the correct LOTO forms and procedures via their mobile device — reducing errors and improving compliance.
